Baptism Request Form
In order to schedule a Baptism, a request form must be submitted and approved by the Church Administrator. Prior to the Baptism date, all parties must meet with the Pastor. If this is for a baby or child, both parents must be in attendance as well as the child.
Once you have summitted your request, you will be contacted by the Church Administrator to discuss dates and times to meet with the Pastor.
